About Care Advocate
Care Advocates are trained to find the resources to help you, from improving your health and well-being to personal care (such as house cleaning, grocery shopping, bill management, etc.), estate planning, and more. Together, over the phone and email, you’ll develop a personal Care Plan that includes trusted resources and education. Care Advocates identify areas you want to focus on, help set goals and provide the expertise to reach them.
How the Care Advocate service works
To use Care Advocate, you must subscribe to a Lively Premium plan for either phones or medical alerts.
Set up a call with your Care Advocate to discuss your objectives and wellness goals.
Receive your personalized Care Plan with trusted resources and tools, all hand-picked for you by your Care Advocate.
Complete the activities in your plan and set up follow-up phone sessions with your Care Advocate as needed.
Common questions
All Care Advocates are licensed social workers with Master’s degrees in the states in which they reside.
Care Advocates can help you with many personal goals, such as:
Health and wellness: They'll help you connect with community events, the right personal trainer, nutritionist and more.
Day-to-day activities: They'll help you find resources for food delivery, household needs, cleaning services and more.
Finances: They'll find the resources to help you with your taxes, budgeting, estate planning and other financial goals.
For certain areas of expertise your Care Advocate may connect you with professionals in specialized practices that can address your questions and concerns.
A Care Plan is a comprehensive document that identifies goals and objectives to be addressed through resource referrals and psychosocial support.
You can change or update your Care Plan as you see fit. It is the roadmap that will help you achieve your goals.
To create your Care Plan, a Care Advisor will ask you some simple screening questions to identify possible areas of opportunity where they can offer assistance. Together, you will determine your personal objectives and wellness goals as well as next steps. About a week after this assessment, you'll receive an email with your personalized Care Plan.
